WHAT IS SUPPORTED LIVING? 
Supported Living is for people with learning disabilities, autism and complex care needs. Supported Living offers an alternative to residential care and empowers individuals to choose the right amount of support, how and when they need it to ensure individuals can lead an enriching and fulfilling life. 
Whatever the need for Supported Living, the overall aim is to make social inclusion a key part of the service, and to ensure each individual is made to feel like a valued member of their community. 
 
The Supported Living homes we can offer as housing solutions are all in a good distance of key amenities, with transport links close by. We are able to provide housing solutions for a variety of different needs and requirements. 
 
Above all, via our extensive experience in providing support ensures individuals can live as independently as possible, in their local community. Working in partnership, co-production and inclusive, with individuals and their families from the very first step of a referral, is imperative to providing not only a safe home and provision, but also a real opportunity for each individual to live a better quality of life in the long term. 
 
Co-Production = Building relationships where we, and the people we support, share power to develop, plan and deliver support together. 
Inclusive = The people we support must be at the very heart of everything we do and be included in every decision we take. We will focus on truly delivering outcomes for them and improving lives. 

REFERRAL PROCESS 

At Ace24 Healthcare we accept referrals from voluntary, statutory and private sectors. Once a referral has been received, an initial assessment will be carried out in order to decide which level of support is required for each individual. This initial assessment will include us using our range of person-centred tools, to enable us to really drill down to what support is necessary, to ensure we get to know each individual, their family and their own personal circumstances. 
 
Through experience, we know the stumbling blocks could be when an individual is ready for discharge from an Assessment and Treatment Unit (ATU), or challenging behaviours have been identified. These types of issues unfortunately traditionally, cause lengthy roadblocks in developing lifelong support for the most vulnerable individuals. By planning for and taking changing needs into consideration at every step of the journey, by working together in total partnership as highlighted above, will ensure Ace24 Healthcare is able to suppport individuals through any challenges.
